VIDEO - CHP SERVES MEMBERS

Community Health Partnership is the main link between our members and their healthcare provider. We are also a major coordinator for working with outside resources to help an individual who would normally be placed in a nursing home, to remain in their own home. This helps decrease fragmentation and duplication of services. It also helps to conserve limited resources, yet provide high-quality care.
