SERVICE AREA

Community Health Partnership, Inc. (CHP) provides healthcare coverage to residents of a five-county service area in west-central and western Wisconsin. These counties include Chippewa, Dunn, Eau Claire, Pierce, and St. Croix. A broad provider network allows members to obtain their care from any of our affiliated providers, hospitals and pharmacies within the service area.
