Community Health Partnership, Inc. Opens River Falls Branch Office

Community Health Partnership, Inc. (CHP) has opened a third branch office located in River Falls, Wisc. Based in Eau Claire, Wisc., CHP is a non-profit, contracted Medicare/ Medicaid care management organization and coordinates health and long-term care funding and services for frail elderly and adults with physical or developmental disabilities. CHP also operates branch offices in Chippewa Falls and Menomonie, Wisc.

The new River Falls office will be housed in the Hillside Plaza building at 186 County Road U, Suite 100, and will occupy a 12,000 square foot suite on the first floor. The office will initially operate as a base for approximately 15 staff serving Pierce and St. Croix counties in CHP’s service area. Plans are for the facility to accommodate up to 50 staff within a year. Designated as one of the state’s regional Managed Care Organizations (MCOs) by the Wisconsin Department of Health and Family Services (DHFS), CHP also provides long-term care services in Dunn, Eau Claire, and Chippewa counties.
