Community Health Partnership, Inc. (CHP) is a non-profit, contracted Medicare and Medicaid care management organization integrating health and long-term care funding and services for frail elderly and adults with physical or developmental disabilities.

CHP’s services are funded by Medicaid and Medicare. Payments are “capitated” which means they are a fixed amount, per member, per month. These capitated payments are generally less than the amount that would be paid to people with similar conditions in a fee-for-service environment. Payments also cover some services that would not ordinarily be covered through Medicare and Medicaid.

Program Options

CHP offers members two program options for their care:

Community Health Partnership (HMO SNP), a Program of Partnership Health Plan, Inc. and Community Family Care. Both programs provide long-term care for frail elderly and adults with disabilities.

Services offered through Community Health Partnership (HMO SNP), a Program of Partnership Health Plan, Inc. coordinate coverage that encompasses all health care, including physician and hospital services, pharmacy and dental services, and long-term care such as personal/supportive home care.

Our Community Family Care program offers several long-term care services. Community Family Care’s coverage does not include primary and acute care or pharmacy and dental services.

Eligibility

Adults will be eligible for our programs if they:

  • Are at least 65 years old or are at least 18 years old and have a developmental and/or physical disability; and,
  • Have long-term care needs; and,
  • Meet functional and financial requirements (Medicaid eligible); and,
  • Reside in one of the following Wisconsin counties: Chippewa, Dunn, Eau Claire, Pierce, or St. Croix. (Service Area Map)
