Community Health Partnership, Inc. (CHP) is a non-profit, contracted Medicare and Medicaid care management organization integrating health and long-term care funding and services for frail elderly and adults with physical and developmental disabilities.

Many adults experience chronic medical conditions, physical disabilities, cognitive concerns, or perhaps most commonly, a decline in the ability to carry out their daily activities. Community Health Partnership, Inc. (CHP) offers a range of services designed to address these situations. Our programs are for adults who would like to live in a home environment, but cannot do so without support services.

Since 1997, we have been successfully providing health care management and long-term care services as a member of the innovative Wisconsin Partnership Program. Our one-stop approach involving an interdisciplinary health care team, helps members with chronic conditions and/or frailty maneuver through the maze of meeting their health and long-term care needs while helping them to live independently and maintain their quality of life.
