Both our Partnership and Family Care programs contract with corporate and single owner facilities that offer residential placements for all eligible populations. Also included in our residential provider network are one and two-bedroom Adult Family Homes which primarily serve adults with developmental disabilities.
Our network of residential providers includes homes licensed by Wisconsin’s Department of Health Services (DHS). Our goal is to provide healthy alternatives for dependent care in the community, emphasizing quality of life issues consistent with standards of accountability and regulatory safeguards.
How to Become a Certified AFH Provider
Residential Development
Community Health Partnership, Inc. (CHP) contracts with a variety of service types to meet the residential needs of our members. We are seeking new providers to broaden our network, so that we are able to match the right services and the right location for each member. As part of the provider network, each home has the opportunity to help our members achieve the highest level of independence as possible, and create a supportive environment that will help that member meet their outcomes. Training Opportunities & Other Educational Resources
Each type of residential provider has general training requirements established by the Wisconsin Department of Health and Family Services (DHS). In addition, the DHS regulatory codes mandate that providers be aware of and meet the specific disability related information that pertains to the members that they serve. CHP provides a monthly training opportunity to each of our owner-occupied Adult Family Homes that will fulfill their general training requirements, as well as a number of training sessions throughout the year. An additional resource and educational opportunity is provided by the Wisconsin Association of Adult Family Care Coordinators Conference which is held each year and features numerous workshops and keynote speakers addressing issues related to Adult Family Home Care. Numerous online training resources are also available for both general core training for licensed homes, as well as disability specific topics.

Owner-operated adult family home networking meetings provide an opportunity for certified adult family home providers to get together to exchange information, network, obtain updates on practices and processes, and learn about other residential opportunities.
