PHARMACY INFORMATION

The Community Health Partnership, Inc. (CHP) Drug Formulary is a list of medications intended for use by our plan physicians and pharmacists. The formulary helps physicians to assess effectiveness of a drug and maintain cost for the plan and our members.

Community Health Partnership, Inc. provides coverage for a limited number of Over-The-Counter (OTC) medications. Over-the-counter drugs require a legal written prescription in order to be covered.
