Partnership (HMO SNP) (CHP) Pharmacy Directory.
 The pharmacy directory provides a list of our network pharmacies. We call the pharmacies network pharmacies because we have made arrangements with them to provide prescription drugs to plan members. A network pharmacy is a pharmacy where members obtain prescription drug benefits provided by CHP. You can search our directory by type of pharmacy, city, state or zip code. Community Health Partnership has contracts with pharmacies that equal or exceed CMS requirements for pharmacy access in your area.
