PARTNERSHIP PROGRAM

Community Health Partnership, a program of Partnership Health Plan, Inc. provides for expanded care management. Member benefits include:

  • health and medical services
  • prescription drugs
  • long-term care services

Continued independence and quality of life are common desires for us and our loved ones. For individuals with chronic health conditions or functional decline, it can be difficult to meet these challenges without assistance.

Our Partnership Program partners with you, local doctors, family, and the community to create a plan of care that emphasizes your independence. As a member in the Partnership Program, you are assigned an Interdisciplinary Team comprised of a nurse practitioner, registered nurse, social services coordinator, a team assistant and other professionals. These staff are trained to coordinate and provide care to meet your needs. Most importantly, your Team gets to know you very well and works closely to support you in your wish to live as independently as possible.

A contract with the Centers for Medicare and Medicaid Services (CMS) along with a contract with Wisconsin Department of Health and Family Services (DHFS) allows the Partnership Program to cover all your Medicare & Medicaid benefits. One of three Partnership Programs in Wisconsin, CHP provides you with the health and supportive care you need to remain as independent as possible and active in the community.

Eligibility Requirements

To enroll in our Partnership Program, you must meet the following eligibility requirements:

  • Be at least 65 years old or be at least 18 years old and have a developmental and/or physical disability; and,
  • Have long-term care needs; and,
  • Meet functional and financial requirements (Medicaid eligible); and,
  • Reside in one of the following Wisconsin counties: Chippewa, Dunn, Eau Claire, Pierce, or St. Croix.

Your county’s Aging and Disability Resource Center (ADRC) will assist you in reviewing your options and will enroll you in the Partnership Program, if that is your choice. Membership is voluntary, and you can enroll or disenroll at any time.

Benefits and Services

Our Partnership Program covers Medicare, Wisconsin Medicaid, and Home and Community-Based Waiver Services. The following are examples of services available through the Partnership Program and our network providers.

In-home services:

  • Nursing care
  • Social services
  • Personal care (assistance with bathing, dressing, etc.)
  • Assistance with medications
  • Meal preparation
  • 24-hour emergency call line

Physician care:

  • Over 170 local primary care physicians throughout Chippewa, Dunn, Eau Claire, Pierce, and St. Croix counties
  • All medical specialties

Other services provided by the Partnership Program include, but are not limited to:

  • Physician and specialty visits
  • Medical, surgical, and intensive care
  • Hospitalization
  • Nursing home care
  • Transportation
  • Nutrition counseling
  • Therapy and rehabilitation
  • Durable medical equipment and supplies
  • Vision, hearing, and dental
  • Prescription drugs and medicines
  • Mental/behavioral health
