The Community Family Care program offered through Community Health Partnership, Inc., provides home and community-based long-term care services. Covered benefits are comparable to those offered through the Wisconsin Medical Assistance Program.

As a Community Family Care member, you are part of a team that includes a social services coordinator, registered nurse, and your own primary care physician. The team works together with you to develop and manage a plan of care.

Your care team will work with you to coordinate all of your health, medical, and long-term care services. This allows you to receive many health and personal care services right in your home. Your team will support you in setting up medical appointments and getting you to the doctor. They can also provide assistance in dealing with confusing medical bills.
