Partners Place Adult Day Services & Activity Center is a bright and vibrant place where your loved one can actively and enjoyably spend the daytime. Our home-like setting is designed for adults with medical conditions, physical disabilities, cognitive concerns, or difficulties functioning at home alone.

The activities help maintain a regular routine and nighttime sleeping pattern.

A “Person-Centered Care” Approach

At Partners Place, our “Person-Centered Care” approach utilizes a philosophy of care that respects and values the uniqueness of the individual, while seeking to maintain and even restore the personal worth of individuals. The concept is carried out by creating an environment that promotes personal worth and uniqueness, social confidence, respect, truthfulness, independence, engagement, and hope.

A plan of care is used to individually tailor activities and services to the current level of memory loss and related needs. This plan of care is written with the help of the staff and family, and is reviewed often to evaluate the goals of the plan.

Nutritious lunches and ongoing health monitoring are provided. Trained health-care workers help participants remember medications; and they provide dignified support with feeding, toileting, hygiene, and mobility. Showers can also be arranged. Participants may attend from one to five days per week.

Enrolling your loved one at Partners Place affords you time to run errands, go to work, visit friends, shop, or fulfill other personal and family commitments. You also know that your loved one is receiving expert care in a safe environment and is occupied with various activities that meet his or her needs, abilities, and interests.

1. What is an adult day services facility?

Partners Place, an Adult Day Services Center, is designed for adults with medical conditions, physical disabilities, cognitive concerns, or those individuals having difficulties functioning at home alone. Partners Place offers a home-like atmosphere where adults spend the day in a group setting, partake in activities, and receive such services as meals, snacks, health monitoring, and personal cares.

2. What kind of funding assistance is available?

Partners Place can be far less expensive than nursing home placement or assisted living facilities. Fees are based on a full day or half-days with lunch and two snacks. A sliding fee scale is available for those who qualify. Many of our participants receive total or partial payment assistance from one of the following:

  • Veteran’s Administration
  • Medicaid Waivers
  • Community Health Partnership, Inc.

Private insurance or other payment options are also accepted.

3. What are some typical adult day services activities?

Participants are encouraged to socialize and participate in group exercises, dancing, singalongs, poetry, drama, word and board games, painting, and various indoor and outdoor activities.

4. What kinds of staff are there at a day services facility?

A Program Director helps create, implement, and evaluate daily activities. A Registered Nurse enables Partners Place to offer a wide range of medical services such as bathing, dressing changes, specialized diets, diabetic monitoring, and medications. Both are helped by day service assistants. In addition to the day services staff, several volunteers provide entertainment and socialization for our participants.
