MEMBERS & FAMILIES

At Community Health Partnership, Inc. (CHP), our programs exist to provide health care coordination and support services to older adults and adults with disabilities who have chronic medical conditions, cognitive challenges, and/or a decline in their ability to function at home.

Our approach is holistic, considering the needs of the “whole” person within the context of each unique situation. We always keep dignity and personal wishes at the forefront of the care plan that is created for each individual.

We do this by partnering with the individual member, family members, local doctors, and other service providers in the community. Together, we aim to:

  • Foster independence and help keep the person at home or in the community for as long as possible
  • Apply preventive healthcare measures, by anticipating problems and concerns in advance
  • Provide direct assistance, advice, and respite for family and other caregivers
  • Emphasize "Member’s Choice" in planning his/her own care
  • Promote "one-stop-shopping" for services
  • Reduce costs by providing outpatient health care interventions
  • Improve and maintain quality of life

A Team Approach to Your Care

At the heart of our programs is the Interdisciplinary Care Team. Comprised of both health and long-term care professionals, they work with the member to make certain services are received in a timely and organized manner.

Help in Managing the System

The care team understands the complex health care delivery system. They act as a guide and advocate for the member to manage and enhance delivery of services. Using a process called the Resource Allocation Decision method, the care team will create a personalized service plan. The service plan will focus on the outcomes desired by the member and the resources needed to achieve those outcomes. We will provide the right services in the right amounts at the right time to support the member’s changing needs.

Voice in Decisions

Our members share and have responsibility in the decision-making process with the care team to help achieve the best overall treatment and support.

Program Options

CHP offers members two program options for their care:

Community Health Partnership, a program of Partnership Health Plan, Inc. and Community Family Care. Both programs provide long-term care for frail elderly and adults with disabilities.

Services offered through Community Health Partnership, a program of Partnership Health Plan, Inc. coordinate coverage that encompasses all health care, including physician and hospital services, pharmacy and dental services, and long-term care such as personal/supportive home care.

Our Community Family Care program offers several long-term care services. Community Family Care’s coverage does not include primary and acute care or pharmacy and dental services.
