Welcome to Community Health Partnership, Inc. (CHP), a
non-profit, contracted Medicare/ Medicaid care
management organization integrating health and long-term
support services for the frail elderly and adults with
physical or developmental disabilities who have chronic
conditions and long-term care needs.

Founded in 1997, we currently serve nearly 3,000 members
through our Partnership and Family Care programs. Our
service area in west-central Wisconsin includes Chippewa,
Dunn, Eau Claire, Pierce, and St. Croix counties.
