Traveling north on US Hwy. 53 (S. Hastings Way), turn right on Fairfax Street (at lighted intersection – Walgreens is located on your right). Go approximately four blocks. Turn left into parking lot adjacent to EastRidge Center. Entrance to Community Health Partnership is located on south end of EastRidge Center.

Traveling south on US Hwy. 53 (S. Hastings Way), turn left on Highland Avenue. (at lighted intersection, Perkins and Burger King restaurants will be on your right. MegaFuels Convenience store is on the left). Go approximately two blocks. Turn right on Fairfax Street. Go approximately 1 block and turn right into parking lot adjacent to EastRidge Center. Entrance to Community Health Partnership is located on south end of EastRidge Center.

Directions to the Community Health Partnership, Inc. Chippewa Falls office located at 475 Chippewa Mall Drive, Suite 418 (map)

Traveling north on Hwy. 124, turn right on Summit Avenue and then right on Chippewa Mall Drive. CHP office is located in the NorthRidge Center complex.

Traveling south on Hwy. 124, turn left on Summit Avenue and follow directions above.

Traveling west on US Hwy. 12/29 (Stout Road), turn left on 550th Street. Turn right on Lookout Road. Stay straight on to Schneider Avenue SE. Office is located on left side of street.

Traveling east on US Hwy. 12/29 (Stout Road), turn right on 550th Street and follow directions above.

Traveling west on I-94, take WI-65 exit (Exit 10 - River Falls/New Richmond). Turn left on WI-65 South. Go approximately eight miles. Turn right on frontage road (County Road U) to U.S. Hwy. 35. (near Harley Davidson dealership). Go approximately ½ mile (you will pass F&K Construction). At the first stop sign, stay on the frontage road and drive across County Road “U” (road curves to left). Our two-story brown brick office is located on right side of frontage road. Drive around to back of building to CHP office entrance.

Traveling east on I-94, take WI-65 exit (Exit 10 - River Falls/New Richmond). Turn right on WI-65 South. Go approximately eight miles. Follow same directions as listed above.

Travel Via Birch Street: Traveling north on US Hwy. 53 (North Hastings Way), turn right on to Birch Street exit. Turn right on to Birch Street at lighted intersection. Travel approximately 1 mile and turn left on Hwy. QQ/Birch Street. Travel approximately 1 mile, turn right on Tower Drive. (Note: Hwy. QQ/Birch Street turn into McKinley Road just before Tower Drive. Go approximately 1 mile. Partners Place is located on left (north) side of Tower Drive

Travel Via US-53 Bypass: Traveling north on US-53 (Eau Claire bypass), take Exit 90 (Hwy. 312/North Crossing). Go approximately ¼ mile, turn right on to County Road Q West/North Crossing. Go approximately ¾ mile, turn right on Peterson Avenue. Go approximately ½ mile. Turn left on Tower Drive. Partners Place is located in the first house on left.
