What are Certified Adult Family Homes?
Certified Adult Family Homes are private residences within the community in which individuals who are 18 years or older and cannot live independently because of aging, developmental, or physical disabilities are placed.
Adult Family Homes provide safety, supervision and stability for individuals and serve as an alternative to nursing homes or institutional care. They are designed to develop and maintain the skills and independence of the individual.
Adult Family Home providers can be single, married, employed or retired and may own or rent their own home or apartment.
How are placements made?
Placements are arranged and supervised through Community Health Partnership, Inc. (CHP). Each placement is mutually agreed upon by the individual and the care provider. The length of stay in the home differs with each placement.
As a provider, who will I care for?
Individuals who reside in an adult family home setting encompass a variety of people with differing needs, skills and limitations, including:
- Frail elderly and elderly with Alzheimer's disease and other dementias
- Adults with developmental or physical disabilities
- Some individuals who may be involved
in adult day care, supported employment or receive respite care
What are examples of services I might provide?
- Above all, you will help an adult live a more fulfilling life by providing a supportive, caring home in his/her own community
- Personal care such as dressing, eating, bathing, grooming and mobility
- Room and board
- Supervision as needed
- Assistance with medication
- Transportation
How will I be reimbursed?
Adult Family Home providers serving individuals enrolled in either CHP's Partnership or Family Care program receive a monthly payment for their service - based on the level of care needed for the individual under their care. Enrolled CHP members must be residents of Chippewa, Dunn, Eau Claire, Pierce, or St. Croix counties.
How to become a CHP Adult Family Home Provider
Call Community Health Partnership, Inc. at 715-838-7420 and speak with the Provider Relations Department. After contacting us, you will need to complete the AFH Provider Application/Change Form and criminal background check as well as undergo a health exam. Personal references are also required. Your home will be inspected to ensure it meets the physical requirements of the program. In addition to the initial certification process, annual training is required. Support for this training is provided by CHP.
