Continued independence and quality of life are common desires for us and our loved ones. For individuals with chronic health conditions or functional decline, it can be difficult to meet these challenges without assistance.
Our Partnership Program partners with you, local doctors, family, and the community to create a plan of care that emphasizes your independence. As a member in the Partnership Program, you are assigned an Interdisciplinary Team comprised of a nurse practitioner, registered nurse, social services coordinator, a team assistant and other professionals. These staff are trained to coordinate and provide care to meet your needs. Most importantly, your Team gets to know you very well and works closely to support you in your wish to live as independently as possible.
The Partnership Program is a Coordinated Care Plan with a Medicare Advantage contract and a contract with the Wisconsin Medicaid program. This allows the Partnership Program to cover all your Medicare & Medicaid benefits. One of three Partnership Programs in Wisconsin, CHP provides you with the health and supportive care you need to remain as independent as possible and active in the community.
Eligibility Requirements
To enroll in our Partnership Program, you must meet the following eligibility requirements:
- An adult age 18 and over with a Physical or Developmental Disability or are over the age of 65
- Live in the service area,
- Financially eligible for Wisconsin Medicaid,
- Functionally eligible as determined by the State of Wisconsin Long-Term Care Functional Screen, and;
- If you are eligible for Medicare you must be enrolled in Medicare Part A, Part B, and Part D
Individuals who are eligible to enroll in the Partnership Program, have both Medicaid and Medicare, and have a diagnosis of End Stage Renal Disease (ESRD) at the time of application are not eligible to enroll. If an individual is diagnosed with ESRD while enrolled in Partnership, the individual is eligible to remain enrolled. Individuals who have Medicaid only and have a diagnosis of ESRD at the time of application are eligible to enroll.
Your county’s Aging and Disability Resource Center (ADRC) will assist you in reviewing your options and will enroll you in the Partnership Program, if that is your choice. Membership is voluntary, and you can enroll or disenroll at any time.
Benefits and Services
Our Partnership Program covers Medicare, Wisconsin Medicaid, and Home and Community-Based Waiver Services. The following are examples of services available through the Partnership Program and our network providers.
In-home services:
- Nursing care
- Social services
- Personal care (assistance with bathing, dressing, etc.)
- Assistance with medications
- Meal preparation
- 24-hour emergency call line
Physician care:
- Over 170 local primary care physicians throughout Chippewa, Dunn, Eau Claire, Pierce, and St. Croix counties
- All medical specialties
Other services provided by the Partnership Program include, but are not limited to:
- Physician and specialty visits
- Medical, surgical, and intensive care
- Hospitalization
- Nursing home care
- Transportation
- Nutrition counseling
- Therapy and rehabilitation
- Durable medical equipment and supplies
- Vision, hearing, and dental
- Prescription drugs and medicines
- Mental/behavioral health
